Smile Makeover 2

Fig 1. The patient’s smile at his first visit.

This patient had badly decayed and broken front teeth which were causing him significant pain and embarrassment. He sought a solution which not only addressed the obvious problems but also restored his teeth to achieve a perfectly natural result.

I decided to restore the upper four front teeth with Emax crowns after rebuilding them together with his upper right canine tooth with composite resin. The remaining teeth were whitened with our Deep Bleaching system.

Our Master Ceramist Ian took great care to mimic the natural characteristics of the patient’s original teeth. Detailed shade maps were made of every part of the teeth to allow the correct porcelains to be used. A total of 12 different porcelains including opalescent and translucent colours were used to make each crown.

The crowns were tried in to check size, shape and basic colour, after which Ian painted each one while it was in the patient’s mouth, thus achieving a perfect match with the adjacent natural teeth.

Needless to say the patient was delighted with the result especially as people he meets never guess that his perfect smile was once so different.

Smile Makeover 1

Fig 1. The patient’s smile at her first visit. Note the excess visible gum. Also the curvature of the lower lip is not mirrored by the edges of the upper front teeth, thus creating a masculine look.

This patient had existing crowns on her upper teeth but was unhappy with the overall appearance of her smile. She showed an excessive amount of gum when she smiled and her crowns were too short, square and bulky. The smile line did not follow her lower lip curvature and therefore looked too masculine and out of place.

Fig 2. Side view pre-op

The treatment plan was firstly to perform a “Gum Lift” whereby the amount of visible gum is reduced by repositioning it higher up. Then the crowns were replaced using the principles of smile design which take into consideration not only the size and shape of the teeth, but also the frame, i.e. the lips, mouth and face. The patient wanted a bright glamorous look and so very high value shades of porcelain were chosen with less translucency.

Fig 3. Gum levels have been corrected with a Gum Lift. New crowns fitted which follow the curvature of the lower lip and are correctly shaped and proportioned for a feminine smile.

The patient was extremely happy with the end result which more than met her expectations, so much so that I have since gone on to treat many of her friends and family.
